What to Bring
- Valid photo identification (driver's license or state ID)
- Insurance card(s) — primary and secondary if applicable
- Referral paperwork (if required by your insurance plan)
- List of all current medications and dosages
- Recent imaging results (MRI, X-ray, CT scan) — CDs or digital copies
- Medical records or surgical reports related to your pain condition
- List of previous treatments you have tried
- Payment method for any copay or deductible
What to Expect at Your First Visit
Arrival & Check-In
Please arrive 15 minutes early to complete any remaining intake paperwork. Our front desk team will verify your insurance and collect any applicable copay.
Comprehensive Evaluation
Your physician will review your medical history, discuss your symptoms in detail, and perform a thorough physical examination. This is your time to share everything about your pain experience.
Diagnosis & Treatment Plan
Based on the evaluation, your physician will explain the diagnosis, discuss treatment options, and work with you to develop a personalized plan. You will have time to ask questions and understand each option.
Next Steps
Depending on your plan, you may schedule follow-up appointments, procedures, or additional diagnostic tests. Our care coordination team will help with insurance authorizations and scheduling.
Complete Your Intake Online
Skip the paperwork on your first visit. Our secure online intake form takes about 10 minutes and helps your care team prepare a personalized treatment plan before you arrive.

Office Policies
Cancellation Policy
We require 24 hours' notice for appointment cancellations. Late cancellations or no-shows may be subject to a $50 fee. We understand emergencies happen — please call us as soon as possible if you need to reschedule.
Prescription Refill Policy
Prescription refill requests require 48 hours for processing. Please contact our office during business hours for all medication-related requests. Controlled substance prescriptions cannot be called in and require an office visit.
Medical Records
Copies of your medical records can be requested through the patient portal or by contacting our office. Please allow 5–7 business days for processing.
